Care Management

AWI case management program ensures your care manager will be with you and support you long after your first introduction. Your caregiver will assist you in navigating difficult topics and complex issues, compassionately address any emotional concerns you or your loved ones may have, and be with you to select your amazing care team. Your caregiver will also help you make short and long-term plans for your in-home care needs.

Your care manager is responsible for supporting you in all aspects of your home care program. They will be there to organize your health assessments so we can provide the highest care possible for your specific needs. Additionally, they will coordinate your continuity of care to make sure you have the perfect match caregiver and a consistent home care team. The care manager will strongly advocate on your behalf for all of your health needs so you know someone always has your best interests at heart and will be with you every step.

In the event you do not have any of AWI caregivers on staff and you have your own carers or another agency involved, we work well with other entities. As your case manager manages your LO care needs, they can also train your team in dementia care needs that arise.

Personal Care

When choosing elder care services for your loved one, you may need to choose between companion care and personal care. Because of this, it's helpful to know the difference between the two services.

Companion Services

Companion caregiving is one of our most popular service, covering a broad range of caregiving duties.

With a home care companion, duties typically include activities such as doing laundry, making meals, and assisting with errands. However, this type of care does not include physical assistance with activities like dressing, bathing, or toileting.

Companion care provides assistance to older adults who wish to live safely and comfortably at home. This type of home care covers a range of basic tasks and activities that many seniors find difficult to perform on their own. In other situations, these obstacles can put seniors at risk of losing their independence. But with companion care, seniors can maintain their routines and continue to age in place.

 

This makes it a great choice for seniors who need a bit of help to remain at home, but who don't require advanced caregiving. 

Companion Care $15 EC/hr.

Personal care

Designed for individuals with chronic health conditions, physical disabilities, or difficulties with strength, coordination, and mobility

Personal care is one of the most common types of in-home care. Generally, this type of care is meant for individuals with chronic health conditions, physical disabilities, or difficulties with strength, coordination, and mobility. Care will typically include a combination of tasks and activities, which are designed to ensure the individual's safety, comfort, and well-being.

One of the most important aspects of personal home care (or private care) is assistance with Activities of Daily Living, also known as ADLs. These activities include things like dressing, personal grooming, bathing, and toileting. Personal caregivers have the skills and expertise required to provide assistance with these tasks, as well as similar tasks such as mobility and transfer assistance.

Personal caregivers can offer a helping hand with a number of other tasks, including assistance with errands, meal preparation, light housekeeping, medication reminders, and other routine activities. Our caregivers also provide individuals with companionship and conversation, helping reduce the risks of senior isolation.

Personal Care $25 EC/hr.

**A Care Manager is included with full-time caregiver services and is responsible for general case management, delegation and training only.
